Acute Hospice Care Inc. has been serving the community with specialized compassionate care. Our staff have been trained on the importance of providing individualized care to our patients and loved ones. Our goal is to be a pioneer in hospice while making a real difference in people’s lives.
Our mission
We strive to create a culture of safety and quality by developing a code of conduct, providing education, encouraging open communication, providing leadership that defines how patients, family/caregivers, visitors, and other members of the community can help identify and manage issues of safety and quality, and implementing changes identified by the annual organization evaluation in order to maintain the culture of safety and quality.
Our core value
We recognize the unique physical, emotional, psychological, and spiritual needs of each person receiving hospice care in the home. We strive to extend the highest level of courtesy, safety and service to patients and family/caregivers, visitors, and each other.
Our priority
The provision of state-of-the-art hospice care services with identified centers of excellence. We engage in a wide range of continuing education for hospice personnel and volunteers, as well as other educational programs for professionals and the public.
Our goals
To teach hospice philosophy and provide a continuum of care which recognizes and responds to the physical, social, spiritual, emotional, and other needs of the patient and family/caregiver.
