ACUTE HOSPICE CARE

Your Comfort, Our Goal

Our team of healthcare professionals, led by our Medical Director, provide the following services at no cost to the patient or family

  • Specialized healthcare, coordinated by your doctor and our hospice team of professionals

  • Medications and equipment related to the terminal illness

  • Nursing visits as needed, 24 hours a day, seven days a week

  • Training for your caregivers on how to keep you comfortable and when to give medications.

  • Grief counseling and spiritual support for you and your family

  • Assistance with bathing, personal grooming, light housekeeping, and other chores (as per care team assessment)

  • Volunteer assistance to provide caregivers a rest and tend to other responsibilities

  • Insurance billing to Medicare, MediCal, and private insurance providers

  • Assistance with advance directives and memorial planning

  • Assistance with difficult discussions with family and friends

  • Education on the expected course of the illness or condition and options for your care

  • 24/7 phone-in nursing advice and extra visits, as needed

  • Frequent assessment of your needs

  • Coordination of community resources as needed

  • Help with reaching a consensus on care options, planning for the future, mending relationships, and easing other emotional and practical difficulties experienced by you and your family

  • Help to cope with spiritual questions and needs at the end of life, either directly or by coordinating services with any spiritual connections you may have

Volunteer Services

Patient & Family Support Volunteers provide a wide range of assistance—from companionship for patients, to respite time for caregivers and much more. Specialized volunteers offer patients their professional skills, such as therapeutic massage, healing touch, and hair care.

Medical Equipment

Hospice care provides a comfortable setting for all the patient’s need. Durable Medical Equipment such as a Hospital Beds, Ambulatory Aides, Respiratory, and more.

24 Hour Services

The hospice agency personnel and services are available 24 hours a day, 7 days a week all year long. Services include a 24-hour clinical team to answer any questions or make visits, emergency medication delivery, physician consulting, delivery of supplies and medical equipment, and spiritual, social, and grief support.

Homemakers & Certified Hospice Aides

Home visiting trained and certified professionals who perform bathing, shampoo, range of motion activities, dry dressing changes, assisting the patient to the bathroom, use of commode or bed pan, assisting with meal preparation, feeding the patient, light housekeeping and/or linen changes.

Medical Supplies

The hospice benefit includes a monthly supply of essential medical supplies for wound care, incontinence, hygiene, diagnosis care, and more.

4 Levels Of Care

Hospice provides various levels of care from routine visits to 24-hour bedside care. The nursing team is trained to complete an ongoing assessment of the level of care to assure patient comfort and also for family/caregiver knowledge and support.
